Here is a pattern I see reported almost never, even though the medical literature has plenty to say about it. A nerve can get entrapped by a small muscular slip well outside the disc, and it can contribute to pain that a routine read blames entirely on the disc.
Most readers are looking at the discs, because that is what the eye is trained to grade. Meanwhile a real, treatable cause of the patient's symptoms sits a few millimeters away, unmentioned.
I am not saying every scan hides a secret. I am saying the reader who only looks where everyone looks will only find what everyone finds.
Detail is not perfectionism. It is the part of the job that changes outcomes.
